4 yr old chokes when he cries

My 4 yr. old has a tendency to choke whenever he starts to cry heavily. If he has an accident or even just has a time out, he seems to choke once he starts to cry. When I tried to get him to sleep in his own room, he threw up his dinner from crying so hard. We have him in a toddler bed in our room because I can't go through watching him work himself into this behavior. I have tried talking to his dr. but can't get any real helpful information. He had acid reflux as an infant, but don't see any problems with his actual eating or sleeping that would lead me to think he still has this problem. I know this is a unique problem, but would love some advice. I have a difficult time with discipline because I'm afraid I'll make him cry and then he'll start to choke. I am praying someone out there has had some experience with this sort of problem.
Thanks for any input other parents can offer.
