I have the same problem and have made it a New Year resolution to fix (we just found out we're expecting another child in Sept.)
Here's my plan -I bought my son a full size bed with the low motion coils (I got a Simmons Beauty rest it should arrive in a few days). I plan to lay with him until he's asleep and then sneak out to my own bed.
I'm not recommending this but a friend suggested a small dose of Benadryl (check with you peds for dosing). She said it will help keep him drowsy and make him less likely to care when you leave. She asked her ped and he told her it would not do any harm. She claims it really made the transition easier - You should probably check with your own ped if you're going to try it.

Good Luck to all of you getting jabbed in the middle of the night with itty bitty toes!
