A/B student-Suddenly starts making D/F

My eleven yr old daughter (will be twelve in May) has always been an excellent student. Now for the last two six weeks report cards her grades have dropped drastically. When asked about it she says she doesnt turn in assignments or do homework because she "forgets" or "she doesnt know". What is happening? She also seems to be fighting with her Dad alot more (she began menstuation in January), and saying she wants to live with me. She lives with her father and I get her every other weekend. Please Help.
