First I would like to say congratulations.
As for the ex-boyfriend. Simply tell him that you had an ultrasound and that he will be the father of twins. It really doesn't matter what he thinks of it. He doesn't have to agree to help out. No matter what he says he is leagally responsible. And that responsibility follows him if he wants it to or not.
As for how you will take care of twins at fifeteen. I won't pretend that being a teenage parent is ever easy and yes having two children will complicate things but you will manage. It sounds like you have a very supportive family and that will probably prove to make all the difference in the world. Once you have those children you will love them so much that you will figure it out.
I don't know where you live but many areas have support groups for teenage mothers. Or you could ask at your clinic about starting one. The support and ideas you get from other people in a situation similar to yours would be wonderful. You might also make some great lasting friends (as teenage mothers often feel isolated from their old group of friends once they no longer have time for the things they used to enjoy).
