I am so surprised the obvious hasn't occurred to someone else. This is a 4 year old. He saw mommy with daddy's penis in her mouth. Four-year-olds consider that the mouth is for eating things. He obviously thinks mommy was literally eating daddy's penis. He wasn't seeing a sexual act. No wonder he screamed, and no wonder he was accepting of daddy but not mommy.

I agree that mommy should talk to the son and explain as best she can what was going on. I think daddy needs to be there in case his absence will make the son not believe mommy is telling the truth.

He saw his mommy as this alien monster who was eating daddy . . . literally. He might think he is next on the menu. Talk to him. He won't understand everything, but if you explain it in relation to how it feels for him to rub his penis (he has, believe me), he will understand that mommy was just making daddy "feel good." Good luck and lock the door next time.
