We had the same problem w/our 4 yr old son. With him, it's all psychological, but we were prescribed Miralax to keep the stool soft. If you get Miralax, be careful about the dosing. We were told to give it to him everyday, but it was too much and caused diarrhea, which scared him to death and caused him to withhold pooping even more. He would hold it for up to two weeks. I don't know how he did it. It caused encopresis, which is basically poop leaking around stool impacted in the rectum. It would stain his underwear all day to the point where I put him back in pullups. We were told that kids with encopresis cannot feel the leaking so they have no idea it's happening until they feel it in their underwear. Finally one day I guess he couldn't take holding it any longer and did a softball sized poop in his underwear. After that he must have realized it wasn't that scary, and he started pooping in pullups on a regular basis. We're now at the point where we're trying to get him to go on the potty, but he's afraid to. He'll still withhold if we don't keep an eye on him. It's an ongoing problem.
