bleeding after bowel movement-anyone?

my 4 month old will sometimes have a spot of blood after a bowel movement (he is on soy formula), anyway today when I picked him up from daycare, they said that he ripped his rectum a bit during a thick and straining bm. he did not cry from it, but i wondered if anyone else had this problem? i am going to give him cereal and want to start veggies next week (he has been on cereal for about 3 weeks) please help
