4 yr.old that has massive mood swings

Please help me! I have a 4 yr. that has major mood swings. I have talked to his doctor about this and he sent us to a behavior specialist. We have been trying stuff they told us, but his acting up is getting worse. He has really good days that I ask myself if this is the same kid. I have tryed just talking to him, time out , taking his fav toys away & he just doen't care. I can't handle this mood swings anymore! I have also noticed his 1 yr sister picking up on his acting up & doing the same. We don't give in & let him have his way either. If he's in trouble he says in trouble. My husband & I don't know what else to do. I have a chemical inbalance & wonder if that's what's wrong, but doctors say no. Please help me, I want my sweet loving little boy back!
